To initiate services, either the individual, guardian, or Support Coordinators may submit a request for services form using this link: https://intakeq.com/new/p8bw9v/5spnba or schedule a meet and greet with us to ask any questions and determine if you would like to move forward with services.
-
Once the request for services with 360 Behavior Consulting is received, we will meet as a multidisciplinary team to discuss how Individual Family Caregiver Training can align with the individual’s ISP goals and develop a person-centered plan for training that is approved by the Department of Behavioral Health and Services (DBHDS).
-
Once the IFCT plan is approved, individuals or family members will meet with their BCBA to establish a specific schedule spanning across two months that includes weekly training and seminar attendance.
